How Does it Work?

Unum’s Critical Illness Insurance helps covered employees and their families offset the financial effects of a catastrophic illness with a lump sum benefit if an insured is diagnosed with a covered critical illness. The Critical Illness benefit is based on the amount of coverage in effect on the date of diagnosis of a critical illness or the date treatment is received according to the terms and provisions of the policy.

Critical Illness Benefits

The benefit for Employees is $30,000. You may also elect Spousal coverage for the same amount. Children up to 26 years of age are automatically covered at $15,000 for no additional cost.

  • You – if you’re actively at work
  • Your spouse
  • Your children – Dependent children until their 26th birthday, regardless of marital or student status

Employee must purchase coverage for themselves in order to purchase spouse or child coverage. Employees must be legally authorized to work in the United States and actively working at a U.S. location to receive coverage. Spouses and dependent children must reside in the United States to receive coverage. Certain states may have limitations.

  • Alzheimer’s Disease – 100% of benefit
  • Coronary Artery Disease – 50% of benefit
  • Heart Attack (Myocarial Infarction) – 100% of benefit
  • Major Organ Failure – 100% of benefit
  • Stroke – 100% of benefit
  • Cancer
    • Invasive Cancer (including breast cancer) – 100% of benefit
    • Non-invasive Cancer – 25% of benefit
    • Skin Cancer – $500

Be Well Benefits

Unum offers an annual Be Well benefit to each covered person on the Critical Illness plan, in which you will receive $50 when you get an eligible screening and submit a claim form to Unum indicating you have completed the screening. These screenings include but are not limited to: cholesterol and diabetes screenings, cancer screenings, cardiovascular function screenings, and annual examinations by a physician, among others. File your claim online with a one-time registration on unum.com or over the phone by calling 1-800-635-5597. Please reference policy #0919782-001.
